Lutron Serena and Sivoia QS Triathlon Shade Recall - United States and Canada

On 2/3/2016, Lutron voluntarily agreed, in cooperation with the U.S. Consumer Product Safety Commission and Health Canada to conduct a recall on certain Serena and Sivoia QS Triathlon Roller Shades due to an impact hazard. When not properly installed, or if placed in a location where the shade can be subjected to external impact, these window shades can become dislodged from their brackets and fall, posing an impact hazard.
Affected roller shades were manufactured between November 2013 and November 2015 and were sold through Home Depot, Lowes, specialty window shading dealers, and online at SerenaShades.com.
Please read the steps below to determine if your shade is part of this recall. If you have one or more affected shades, please follow the instructions provided below the identification steps.
How to identify an affected shade
To find out if a shade is included in this recall, follow the steps below:
  1. Locate the shade label.
    On battery powered shades, the label will be located in the battery rail above the roller tube or else on the back of the shade fabric wrapped fascia. If your shade does not have a fascia, the battery rail can be rotated forward to expose the label. If your shade has a fascia, both potential label locations can be accessed by pulling out and down on the bottom of the fascia (see image below).
    On wired power shades and manual shades, the label is located on the back side of the fascia, or else on the support rail above the shade tube. Both potential label locations can be accessed by rotating the fascia up (see image below).
    Note – if your shade does not have a battery rail as shown in the first image or else a support rail as shown in the second image, it is not part of this recall.
  2. Identify your shade family name. The family name is located on the top left of your shade label. If the family name is either Serena or Sivoia QS Triathlon, proceed to the next step. If not, your shade is not included in this recall.
  3. Identify your shade model number. This should be located just below the family name. If your shade model number matches a model in the following list, proceed to the next step. If not, your shade is not affected by this recall.
    • QSFRJ-S2A13-B or QSFRJ-S2A13-BN
    • QSFRJ-S2A13-PS or QSFRJ-S2A13-PSN
    • SYRJ-S2A13-B or SYRJ-S2A13-BN
    • SYRJ-S2A13-PS or SYRJ-S2A13-PSN
    • SMR-S2A13 or SMR-S2A13-N
  4. Identify your shade date code. This should be located near the top on the far right of the shade label as shown in the image below. If your model number starts with a W or an X, your shade is affected by this recall. If your model number starts with a Y followed by a number less than or equal to 46, your shade is affected by this recall. If your date code does not match the above criteria, your shade is not affected.
